Application process

  • Download the Client Contract by clicking here.
  • Read the contract carefully, and answer all of the questions
  • Discuss the collection with your Obstetrician or midwife - most will have done collections before, but we need them to sign the Consent page
  • Choose a payment option. If you need assistance choosing a plan to best suit you please give us a call

  • Completed forms can be sent by post or fax them on (03) 9820 5226 (but you will need to mail the originals in as well)
  • We will contact you to confirm receipt of your application, and to arrange delivery of the 'Blood Collection Pack' (no earlier than 34 weeks gestation)
